LIABILITY RELEASE
By signing this form I understand that the General’s Soccer Academy, staff, and the Anthony Wayne School District will assume no responsibility or liability for accidents or injuries, or for medical, dental, or other expenses incurred as a result of any instruction or application of instruction of the applicant, or contact with another player(s) or object. I hereby authorize the Generals Soccer Academy to act for me in any emergency situation requiring medical attention.
