I hereby grant C.E. Byrd High School Beta Club (“the School”) permission to use the photograph submitted on this website.

I understand and agree to the following:

Usage: The photograph may be used in billboards, posters, social media posts, website content, and other promotional materials related to the School’s billboard campaign.
Duration: This consent is granted for the duration of the billboard campaign and related promotional activities, which may extend beyond the initial display period.
No Compensation: I understand that I will not receive any monetary compensation or other form of payment for the use of the photograph.
No Approval Rights: I understand that I will not have the right to approve the final design or layout of the billboard or other promotional materials.
Release of Liability: I release the School, its employees, agents, and representatives from any and all claims, demands, and liabilities arising out of or in connection with the use of the photograph.
Ownership: I confirm that I am the owner of the photograph or have the necessary rights and permissions to grant this consent. If the student is under 18, the parent or guardian is confirming they have the legal right to give this consent.
Revocation: I understand that I may revoke this consent in writing at any time by contacting [Designated School Contact/Department] at [Contact Information]. However, revocation will not affect materials already published or in production.
Student's image: I understand that the student's likeness will be visible in the photograph, and that the photograph will be displayed publicly.

Signature:
