By clicking this box, I am acknowledging that I am granting permission for my child to participate in this educational field trip, and understand the conditions and limitations in connection with this education field trip that include the following:
A. It is expressly understood and agreed that the student and parents assume risk of harm, injury, or loss which the student may incur during the course of this trip and that, as a result, the undersigned expressly waives and releases the Avon Board of Education from any and all such claims or liabilities.
B. It is further understood and agreed that the student shall comply with all rules and regulations of the Avon Local Board of Education, including suggestions, recommendations, rules and regulations of chaperones and staff members, in all matters pertaining to the program or personal conduct.
C. I understand that the emergency information provided to the school that is on file in PowerSchool will be used in the event of an emergency.
D. There will be NO REFUNDS.
