MEDICAL TREATMENT CONSENT
I, the parent/guardian of the following participant, recognize that as a result of participation in Powder Puff Football practices, game and or related activities that injuries can occur and result in emergency treatment. I further recognize that school personnel may be unable to contact me for my consent for emergency medical care. I do hereby consent in advance to such emergency care, including ambulance transportation, and/or hospital care as may be deemed necessary under the then existing circumstances. I will assume the full expenses of such care not covered by my insurance company. The Anchor Bay School District is not held liable for injury to the participant or cost of medical expenses.
ANCHOR BAY HIGH SCHOOLPOWDERPUFF FOOTBALL PARTICIPATION - PERMISSION SLIP
I give permission to my child to participate as a Player in the Powder Puff Classic. I verify my child is in good health and has had a physical within the last 12 months. S/he is free to participate without restrictions. S/he will abide by the Anchor Bay High School Code of Conduct, the Athletic Code of Conduct, and will follow all school district rules governing conduct at extra-curricular activities. In addition, s/he will follow the specific rules that are outlined on the PowderPuff Participation Rules sheet as well as the PowderPuff Game rules which pertain to PowderPuff football.
We have read and understand the PowderPuff Participation Rules, and agree to follow them. Signing (using the electronic signature) is acknowledging that you have read and understand the rules. You also understand that Anchor Bay Schools or any club associated with Anchor Bay High School is not responsible for any loss or damage to any personal property during practices, the game or any event associated with the Powderpuff game.
