About Lady Braves Soccer Camp
Lady Braves Soccer team will be
hosting, this year, a girls only youth camp K-8. This is a fun and active camp
led by our Olentangy coaches and current student athletes.
This camp is an opportunity for
soccer athletes from all levels to practice soccer specific skills and for
those new to the sport to learn and grow in the game of soccer. We will
incorporate scrimmages, skills sessions and fun games.
We look forward to meeting your
athletes and spending 2 days of fun, growth and learning!
This camp is open to all participants.
Athletes from All Olentangy schools are welcome.
All camp proceeds benefit our team and student athletes managed by The Lady Braves Boosters.

By registering for camp you agree to the following as "the undersigned"
WAIVER
The
undersigned, on behalf of himself/herself and on behalf of his/her
child ("the registrant"), recognizing the possibility of physical injury
associated with the following activity: SOCCER, and I as
parent/guardian offer the registrant for participation in said activity,
assumes and accepts full responsibility for any and all liability
claims arising from this activity, and releases, discharge and/or
otherwise indemnifies OLENTANGY YOUTH SOCCER CAMP, the Olentangy Local
School District and all its employees from liability for any claim by
or, on behalf of the registrant as a result of the registrant's
participation in the activity.
CONSENT FOR EMERGENCY TREATMENT
In
the event that reasonable attempts to contact me at the phone numbers
submitted in this form have been unsuccessful, I hereby give my consent
for (1) the administration of any treatment deemed necessary by my
preferred Physician or preferred Dentist, or in the event that the
designated preferred practitioner is not available, by another licensed
physician or dentist; and (2) the transfer of the child to the preferred
hospital or any hospital reasonably accessible.
