To submit a facility use payment follow these instructions:

1.     Click orange submit payment button located in the bottom right corner of this page

2.     Sign in, create an account or check out as a guest

3.     Select your invoice number by selecting 1 in the drop down box      

4.     Scroll to the bottom of the page

5.     Enter general information and click the continue button.

5.     Follow prompts to complete payment.
