ALUMNI TRANSCRIPT REQUEST – Except as provided by law, records are only released upon prior written consent of the student or the student’s legal guardian.* Per Board policy transcripts cannot be released until all fines and fees have been paid. Transcripts are typically sent within 5 school days from the date of receipt and filled on a first come first serve basis. During high traffic months there could be delays in processing your request. Please allow adequate time to process your request.
There is a non-refundable $3.00 fee for each record request form completed.
It is your responsibility to know the type of transcript and delivery method accepted to where you are having it sent as well as to provide the correct and complete delivery information.
CURRENT STUDENTS-If you are a current Cloverleaf High School student DO NOT use this ALUMNI TRANSCRIPT REQUEST site. Current CHS Students can request transcripts through their SCHOOLINKS accounts. If you are not sure how to do this, please contact your School Counselor.
IMMUNIZATION RECORDS-Cloverleaf High School only maintains immunization records for 5 years. If your graduation year is 5 or more years past we will not be able to obtain immunization records for you.
DIPLOMAS-Diplomas are one of kind documents that are received (when all requirements are met) after the commencement ceremony. Cloverleaf High School does not keep copies of diplomas. The high school transcript is the official record of achievement.
*If you are a service center, college, or other entity requesting a transcript on an alumni's behalf, you must email the records release authorizing you to receive the transcript on the alumni's behalf to heather.osborn@cloverleaflocal.org before the transcript will be released to you.

Official Sealed transcripts can be sent ONLY through USPS mail service or picked up. Official Sealed transcripts CANNOT be sent through email.
